Respondent was assessed a civil penalty of $20,800 for four violations of the HMR when it filled a DOT specification cylinder that is overdue for periodic requalification, with a hazardous material, and then offered for transportation in commerce; overfilled a DOT-specification cylinder with a hazardous material, so that the effectiveness of the cylinder was compromised, resulting in release of the contents (propane) to the environment; failed to provide initial training to hazmat employees; and failed to label a non-bulk package containing a hazardous material.
