Applications may be submitted:

1. Online through the Competent Authority (CA) portal, accessible using the Hazardous Materials Portal;
    
2. By email to approvals@dot.gov; note the request type in the email (e.g., Self-Reactive Material or Organic Peroxide Competent Authority Approval Request (Application, Renewal, or Modification));
    
3. By mail, see 49 CFR 107.705(a)(1) for the full address.
    
4. By facsimile (fax) at (202) 366-3753 or (202) 366-3308.

Organic Peroxide and Self-Reactive Materials applications for a CA approval must include the following:

1. Request letter with information as described in 49 CFR 107.705(a) and (b).
   1. Identify the section of the chapter under which the application is made. Typically, this is 49 CFR 173.128(d) for organic peroxides and 49 CFR 173.124(a)(2)(iii)(D) for self-reactive materials.
       
   2. Contact information (company, name(s), street and mailing addresses, email, phone) of the person on whose behalf the application is made and, if different, the person making the filing.
       
   3. United States Designation of Agent for Service information (for foreign applicants) prepared in accordance with 49 CFR 105.40.
       
   4. A description of the activity for which the approval is required.
       
   5. The proposed duration of the approval.
       
   6. The transport mode (or modes) affected, as applicable.
       
2. Test Reports and all relevant data, including:
   1. Physical state (i.e. liquid or solid).
       
   2. Control and emergency temperature, if any, based on the material's self-accelerated decomposition temperature (SADT) (see 49 CFR 173.21(f)).
       
   3. Generic type as determined by the testing protocol from Figure 20.1 – Flow chart scheme for self-reactive substances and organic peroxides from the UN Manual of Tests and Criteria (IBR, see 49 CFR 171.7). 

In lieu of Test Reports, an approval issued by the competent authority of a foreign government in accordance with 49 CFR 173.128(d)(2) (for organic peroxides) or 49 CFR 173.124(a)(2)(iii)(D)(2) (for self-reactive materials).

See 49 CFR 107.705 to ensure all required applicable information is included before submission. Requests for confidential treatment must be marked in accordance with 49 CFR 105.30.

Renewals and Modifications of Current CA Approvals:

• Renewal applications must include all originally submitted required documentation (request letters, test reports, etc.), even if the formulation has not changed, in accordance with 49 CFR 107.705(c).
   
• Submit online via the CA portal or email approvals@dot.gov, specifying the request type in the subject line (Renewal or Modification).
   
• If a renewal is filed at least sixty (60) days before the expiration date, operations may continue under the current approval's existing terms until final administrative action is taken (e.g., approval, denial), in accordance with 49 CFR 107.705(c).
