Respondent was assessed a civil penalty of $6,435 for three violations of the HMR when it represented, marked, and certified DOT specification compressed gas cylinders as having been successfully retested and requalified for continued service in accordance with the HMR, when its calibrated cylinder exhibited permanent expansion (violation No. 1), when its retest records did not contain all the required information (violation No. 2), and it allowed an employee to perform functions subject to requirements in the HMR when the employee had not received general awareness/familiarization or security awareness training or recurrent function-specific training (violation No. 3).
