Respondent was assessed a civil penalty of $6,000 for two violations of the HMR when it offered for transportation, and transported, in commerce, a hazardous material (UN1072, Oxygen, compressed, 2.2 (5.1)), and others, while failing to prepare a complete shipping paper that included the subsidiary hazard class and the proper shipping name; and when it filled a hazardous material (UN1075, Liquefied petroleum gas, 2.1) in cylinders that were overdue for periodic requalification.
