Respondent was assessed a civil penalty of $7,875 for three violations of the HMR when it offered for transportation, and transported, in commerce, a hazardous material (RQ, UN2916, Radioactive material, Type B(U) package, 7), at an activity requiring a written security plan while failing to have one; while failing to create and maintain records for each hazmat employee training consisting of general awareness/familiarization, function-specific, safety, and security awareness training; and while using a shipping description that is not in the required sequence on a shipping paper.
