Respondent was assessed a civil penalty of $4,600 for four violations of the HMR when it represented, marked, and certified DOT Specification compressed gas cylinders as having been successfully requalified for continued service in accordance with the HMR when it failed to maintain complete and accurate records of calibration and cylinder requalification (violation No. 1), it failed to hold a valid requalification identification number (RIN) (violation No. 2), it failed to create and retain records that its retest operator had received hazmat employee training (violation No. 3), and it failed to mark cylinders with a RIN in between the month and year of retest and an S for designation of the proof pressure test (violation No. 4).
