Respondent was assessed a civil penalty of $12,650 for four violations of the HMR when it offered for transportation and transported in commerce a large bulk quantity of a hazardous material, "UN1075, Propane, 2.1 ", while failing to develop a written security plan; offered for transportation in commerce a hazardous material, while failing to provide each hazmat employee with training consisting of general awareness/familiarization, function-specific, safety and security awareness; represented, marked, certified and offered for transportation in commerce DOT specification cylinders as having been successfully requalified in accordance with the HMR, while conducting visual inspections utilizing an expired requalification identification number (RIN); and represented, marked, and certified DOT specification cylinders as having been successfully requalified in accordance with the HMR, while failing to maintain the prescribed information on a record for each cylinder requalified or visually inspected.
