Respondent was assessed a civil penalty of $15,500 for three violations of the HMR when it failed to provide shipping papers, placarding, and emergency response information for the shipment of hazardous materials, thereby creating an undeclared shipment of hazardous materials; allowed employees to perform functions subject to the requirements of the HMR, while failing to keep records of providing each of those employees with general awareness, function-specific, safety, and security awareness training; and offered for transportation, in commerce, a quantity of hazardous material requiring placarding of the transport vehicle while failing to register as an offeror of hazardous materials with PHMSA.
