Respondent was assessed a civil penalty of $8,640 for six violations of the HMR when it represented, certified, and marked DOT specification cylinders as having been successfully requalified without holding a current RIN (violation No. 1); offered for transportation in commerce a hazardous material without providing an authorized emergency response telephone number on its shipping records (violation No. 1); represented, certified, and marked DOT specification cylinders as having been successfully requalified, while failing to maintain complete and accurate daily records of requalification (violation No. 3); represented, certified, and marked DOT specification cylinders as having been successfully requalified, while failing to have a copy of the applicable portions of the HMR pertaining to cylinder inspection; failing to have the CGA pamphlets for visual inspection; and failing to have a copy of its special permit covering a tested cylinder (violation No. 4); offered for transportation in commerce hazardous materials while failing to provide employees with recurrent hazardous materials training in general awareness and security awareness (violation No. 5); and offered for transportation in commerce hazardous materials without a properly executed shipping paper by failing to list the hazard class or listing an incorrect hazard class for the material (violation No 6).
