Respondent was assessed a civil penalty of $12,925 when it committed two violations of the HMR when (1) it represented, marked, and certified ICC specification compressed gas cylinders as qualified for continued service in accordance with the HMR, when Respondent did not hold a requalification identification number (RIN), nor had it performed the required tests, and (2) when it failed to provide hazardous material employee training to a hazardous material employee.
