Respondent was assessed a civil penalty of $18,000 for six violations of the HMR when it offered for transportation, in commerce, Class 8 hazardous materials in non-bulk packages while failing to properly label the packages with a primary corrosive hazard label; offered for transportation, in commerce, liquid hazardous materials in specification packagings marked 1H2/Y200/S/17/USA/M5884, which are certified for solids; offered for transportation, in commerce, a hazardous material (UN1814, Potassium hydroxide, solution, 8, PG II), accompanied by a shipping paper that failed to include the proper shipping name, hazard class, or packing group; allowed employees to perform a function subject to the requirements of the HMR while failing to provide initial hazmat training (general awareness, function-specific, safety, and security awareness); offered for transportation, in commerce, a hazardous material in a non-bulk package, marked with an incorrect proper shipping name; and offered for transportation, in commerce, hazardous materials in non-bulk packages that were marked with identification numbers not meeting the minimum height requirement.
