Respondent was assessed a civil penalty of $5,250 for three violations of the HMR when it represented, marked, and certified a DOT specification cylinder as having been successfully requalified while failing to comply with eddy current examination requirements for DOT 3AL cylinders manufactured of aluminum alloy 6351-T6; represented, certified, marked, and offered DOT specification packages while failing to notify of any change in facility name, address, ownership, management, equipment, or testing personnel within twenty (20) days of change; and represented, marked, and certified a DOT specification cylinder as having been successfully requalified while failing to keep complete and accurate records of cylinder reinspection and retest, specifically, failure to keep accurate test records.
