Respondent was assessed a civil penalty of $4,875 for four violations of the HMR when it offered lighters, a flammable gas hazardous material, for transportation in commerce (1) accompanied by shipping papers that failed to properly describe the material and to include the approval number for the lighters; (2) in packagings that were not marked with the approval number for the lighters; (3) when these shipments were required to be placarded but Respondent had not registered with PHMSA and paid the registration fee; and (4) when Respondent¿s employee who performed a function subject to requirements in the HMR had not received recurrent hazmat employee training or security awareness training.
