Respondent was assessed a civil penalty of $13,280 for four violations of the HMR when it represented, certified and marked that cylinders have been successfully requalified, when its requalifier identification number was expired; transported a bulk quantity of hazardous materials while failing to develop a required security plan; failed to provide general awareness/familiarization, function-specific, safety, and security training to hazardous materials employees; and failed to retain a copy of shipping papers while also failing to indicate total hazardous material quantity.
