Respondent was assessed a civil penalty of $61,400 for four violations of the HMR when it represented, marked, and certified DOT specification and special permit cylinders as having been requalified in accordance with the HMR after May 3, 2007, when its RIN was suspended (violation No. 1), and between April 2006 and April 2007, when it failed to calibrate its test equipment as required (violation No. 2); it failed to test various cylinders at the minimum test pressure (violation No. 4); and it failed to maintain complete and accurate records of calibration and retest (violation No. 5).  One alleged violation dismissed.
