Subject: Information: Post Accident Drug Test Report

The Independent Safety Board amendments Act of 1990 (Public Law 100-691), was signed into law on November 28, 1990. The Act includes a provision requiring the head of each Department of Transportation (DOT) agency to send a report to the Secretary on any post-accident tests performed on DOT employees. The report is to provide specific information on the amount of time required to complete specimen collection for any employee determined to be reasonably associated with the accident/incident.

Use of the attached form is mandatory for complying with this requirement. This form has been designed to collect the specific information that is required by Public Law 100-691.

The collector's travel time to the collection site is required. This information should be available from the collector. If you encounter problems obtaining this information from a contract collector, please contact this office and we will assist you in getting the information.

A separate form must be completed for each specimen collected.

Completed forms must be transmitted to this office no later than three (3) working days following the specimen collection.
