Respondent was assessed a civil penalty of $750 for three violations of the HMR when it loaded and transported a DOT-SP 9166 cargo tank with a hazardous material (UN2014, Hydrogen, peroxide, aqueous solutions with more than 40 percent but not more than 60 percent hydrogen peroxide (stabilized as necessary), 5.1, PG II); it accepted for transportation, and transported, a hazardous material (UN2014, Hydrogen, peroxide, aqueous solutions with more than 40 percent but not more than 60 percent hydrogen peroxide (stabilized as necessary), 5.1, PG II), without shipping papers; and it offered for transportation, and transported, a hazardous material (UN2014, Hydrogen, peroxide, aqueous solutions with more than 40 percent but not more than 60 percent hydrogen peroxide (stabilized as necessary), 5.1, PG II), in a DOT-SP 9166 cargo tank while failing to prevent an adverse reaction with residue of a hazardous material (UN3264, Corrosive, liquid, acidic, inorganic, n.o.s., 8, PG II (cupric chloride, hydrochloric acid).
