Dear Mr. Matlovsky:

This is in response to your letter of January 24, 1974, in which
you request statistical data concerning pipeline ruptures in the
western States.

We are enclosing: (1) a copy of Part 191 regarding the leak
reporting requirements; (2) leak reporting forms DOT F 7100.1
and DOT F 7100.2, (applicable for sections 191.9, 191.13, and
191.15); and (3) a summary of 1972 data obtained from forms DOT F
7100.1 and 7100.2 from Region 5 which includes the States of
Alaska, Arizona, California, Hawaii, Idaho, Nevada, Oregon, and
Washington.
